Urban Medical has been selected to participate in a new Medicare demonstration project intended to track the care of chronically ill Medicare patients. The goal of the three-year project, which begins this summer, is to support the adoption of health information technology and facilitate the care of chronically ill patients in physician practices.
For the demonstration, Urban Medical will use its new electronic medical record (EMR) system to track and report our treatment of Medicare patients with diabetes, congestive heart failure and coronary artery disease. We hope this will help providers ensure that patients receive all of the treatment recommended for their diagnosis and condition. Providers will also use the EMR to make sure that patients receive the right preventive care measures on time, like mammograms and flu shots.
We are now six months into the new EMR system, and we would like to thank you for your patience and support during the transition period.
