Judy Sweeney provides compassionate, high quality care for adults as they age. Judy provides health care to patients in a number of skilled nursing facilities and on-site care to residents of an elderly housing facility. She is very interested in bringing health care directly to elders to allow them to age in place and maintain independence. Judy is also very active in the education of future healthcare providers. She serves as preceptor for nurse practitioners in training and participates in the training of medical students, interns and residents.

Urban Medical is a nonprofit organization providing innovative and compassionate responses to the primary care needs of the elderly, chronically ill and underserved. For 30 years, we have focused exceptional effort on care for the most vulnerable members of our community – the elderly, chronically ill and medically complex.
