Susan Kuriakose offers compassionate, high quality primary care for adults in post-acute settings. During a patient's short term rehabilitation stay after a hospital admission, Susan also focuses on coordinating all disciplines so that the patient may return to the community as soon as possible.

For long-term care residents facing chronic disease, Susan strives to maintain the resident's optimum function and comfort.

Urban Medical is a nonprofit organization providing innovative and compassionate responses to the primary care needs of the elderly, chronically ill and underserved.
