Fund for the Future of Primary Care

As the numbers of elderly and chronically ill increase, primary care is key to an affordable system that delivers health care of high quality. Today, primary care is in crisis. Fewer physicians choose to practice primary care and patients find it difficult to access. Urban Medical has an important role to play in this crisis. The Fund for the Future will allow Urban Medical to provide and advocate for a model of care that attracts and sustains both health care providers and patients. The Fund will be used to:

• Expand the capacity of Urban Medical to innovate and disseminate best practices in caring for the elderly and chronically ill;
• Recruit the best physicians and nurse practitioners into primary care practice;
• Leverage our 30-year track record as a leader in primary care and assume a growing national advocacy role.
